Take on hyena form before you speak with her. Also, becoming the hyena matriarch first should help too.

Actually, there's a few variations depending on your past with the gang. Any of these results will work for a piercing: being the Matriarch, having lost your first challenge attempt, having lost both challenge attempts, or just being a Hyena (body infection). Any of those will work, all with different results.
