Vanessa fully transformed into a centaur, and she gave birth to the foal. Supposedly, talking to her after this may trigger the quest to save Meredith, but I've talked to her over and over, and it hasn't come up. Is there something else I should be doing?

It's a random event. Try leaving the bunker and doing a few things, then come back and talk to her again.
